Integrated Primary Care Team Training

At the heart of Southcentral Foundation’s Nuka System of Care is the belief that the relationship between care team and the patient is the most important tool for managing chronic disease, controlling healthcare costs, and improving the overall health and wellness of our patients.

By embracing a patient-centered system of integrated healthcare delivery, our multidisciplinary team approach means that team members are solidly grounded in the technical day-to day delivery of care and strong interpersonal and communication skills.

In this course, learners will explore and practice these key skills needed to be successful in Southcentral Foundation’s collaborative care model. The training experience explores relational styles and the impact of different work style energies on team dynamics. In addition, learners will develop a personal toolbox that facilitates communication, self-awareness, navigating resistance, and more to successfully managing change and work in an integrated care team.

Cancellation Policy

Email cancellations to scfnukainstitute@scf.cc. Cancellations made 5 weeks or more before the date of the training will receive a full refund minus a $250 administrative fee. Cancellations made less than 5 weeks, but more than 3 weeks before the start of the training will receive a 50% refund. Cancellations made less than 3 weeks before the start of the training are non-refundable. To transfer registrations to another person, the original registrant should email the replacement attendee’s name and contact information to scfnukainstitute@scf.cc.
